The Employee and Family Assistance Program (EFAP) provides confidential counselling assistance to government employees and their families for work-related and personal problems

These forms are available for use by public service employees. Please note that not all employee forms are listed here. Some forms may be available through your department.

The Chair of the Public Service Commission requests employees seek approval of any outside employment, as required by the “Saskatchewan Public Employees – Conflict of Interest Guidelines." 

When an employee is seeking approval for vacation leave, scheduled days off, leave without pay (3 months or less), leave without pay (in excess of 3 months), maternity leave, out of province travel or other type of leave, it is receommended that the employee fill out this form and have a specified individual approve it.  Click here for the PDF version.

Found in the HR Manual - PS901-2. This policy provides for an employee leave to work on further education. The application form is found on page 6 of the policy. Please note that employees may choose to use vacation, banked or EDO time to work on education, as described in PS901-3 in the HR Manual.

Employees and potential employees may choose to self-declare as a member of an equity group. Use this form to do so. Submit it either to your department's human resource branch or the Public Service Commission.

Form for managers to use once an employee has completed his or her initial probation. The form indicates whether the employee has passed probation. Click here for the PDF.

This form is for human resource professionals to use when an employee asks to be included on the re-employment list. HR professionals can also use it if a department chooses to place an employee on the list.
The form required for a supervisor to request that an in-scope employee's term be extended.

Form to use when an employee is looking after duties of an employee with a higher classification

Found in the HR Manual - PS901-1. Employees can request approval for an allowance to assist with continuing education.  Click here for the PDF version that also contains the policy for the application. The application form is on page 4 of the PDF.

Found in the HR Manual. The Variable Work Hours Policy will allow employees to voluntarily reduce their hours of work to a minimum of 50 per cent of the regular full time. Scroll to page nine for the form.

For in-scope field employees. This form provides an employee's consent for the overtime provisions in the Collective Bargaining Agreement to be waived.
